Case Manager Case Managers are responsible for creating a supportive, efficient, and client focused environment by focusing on the behind-the-scenes operations. In addition Case Manager\'s are also a resource for all Lead Youth Support Workers and Youth Support Workers. Their main goal is to ensure that each staff member is confident and competent in their own skill set and knowledge of the site that they are working at by providing support, resources, trainings, incident debriefs, efficient scheduling, following best practices, keeping accurate documentation, facilitating team building initiatives, and ensuring all policies and procedures are implemented. The Case Manager role reports to the Regional Youth Support Manager. Key Duties and Responsibilities:
  • Leads assessment and implementation of goal setting and program planning such as Individualized Support Plans for persons supported.
  • Evaluate individuals needs and develop short-term and long-term plans to meet such needs with the active participation of the individuals and their personal networks (both professional and non-professional).
  • Support Lead Youth Support Workers and Youth Support Workers through various programming, performance management, team building initiatives, coaching, check-in meetings, foster positive workplace environment, and provide resources such as training and benefits information.
  • Oversee employee performance management, hiring, disciplinary procedures, site orientation for new staff, on-going coaching, manage staff clinical competencies when applicable, ensure staff are adequately trained, ensure employee documents are up to date, facilitate incident debriefs, on-going check-ins and employee evaluations.
  • Oversee the residence daily operations - budget, medication tracking, other expenses, staffing, scheduling, liasing with community partners, house maintenance, vehicle maintenance when applicable, ensure house continuously meets Social Development\'s Environment of Care standards, organize periodic public health and fire safety inspections, monitor employee performance and sick days, and monitor shift notes and incident reports to ensure accuracy.
  • Develop a recreation plan for individual(s) in care.
  • Accompanies individual in care to medical appointments and any other appointment as determined by their Social Worker.
  • Recognizes, analyzes, and deals with potential emergency situations and takes a proactive approach in consultation with direct supervisor and clinical team (if applicable).
  • Ensure health and safety standards are being maintained.
  • Attends to the physical, emotional, and medical needs of the children/youth.
  • Ensures that all required documentation is complete and accurate.
  • Complete weekly reports for stakeholders.
  • Manage, ensure accuracy, and approve time-sheets as set out by our payroll procedures.
  • Ensure proper document keeping procedures such as employee files, house related files, training certificates, and any related important documents.
  • Update and individualize policies and procedures as needed for the home.
  • conduct staff meetings bi-weekly.
  • Conduct or organize training and/or support initiatives for staff.
  • Conduct weekly home inspections to ensure health and safety issues are kept to a minimum.
  • Participate in bi-weekly House Manager meetings with Regional Youth Support Manager.
  • Other duties as required by the Regional Youth Support Manager and Director of Operations.
Education and Qualifications:
  • A minimum of 2 years experience working with youth with developmental disabilities and/or physical/medical/behavioural support needs or an equivalent combination of education, training, and experience.
  • Proficient in Microsoft Word and Outlook, email and cloud-based systems.
  • Clear Criminal Record Check with Vulnerable Sector Screening.
  • Clear Social Development Check (will be done in house).
  • Valid First Aid and CPR certification.
  • Reliable transportation.
  • A completed college diploma or university degree in human services field (asset).
  • Food safe certification (asset).
  • Current NVCI training (asset).
  • Previous supervisory experience (asset).
