What you will be doing

PRIMARY DUTIES AND RESPONSIBILITIES:

  • Provide support to patients and health care providers, including but not limited to the enrolment of the patient into the Patient Support Program for the prescribed treatment for the Rare Disease;
  • Demonstrate in depth knowledge of rare disease program offerings, including the pathophysiology of the disease/product, therapeutic area, disease process, and mechanism of action;
  • Collaborate with the Reimbursement Specialist, Rare Diseases and the physician to secure reimbursement for their patient\xe2\x80\x99s therapy;
  • Liaise with health care providers, Specialty Pharmacies and infusion clinics, and other agencies as required, to coordinate infusions, drug delivery and resolve issues as they arise;
  • Telephone support including but not limited to maintenance calls, adherence support, on-going patient follow-up, data collection and therapy disruption/discontinuation monitoring;
  • Reporting of Adverse Events as per the Manufacturer\xe2\x80\x99s requirements;
  • Prepare for and attend specialized training sessions for specific rare disease therapies provided by the pharmaceutical team;
  • Liaise with patient associations and advocacy groups;
  • Establish and maintain partnerships with pharmaceutical team;
  • With the managers, participate in the training process of new case managers, including but not limited to shadowing & call monitoring;
  • Provide feedback regarding current protocols, databases, processes, inter-departmental relationships;
  • The Case Manager, Rare Diseases will also be assigned other duties and tasks as required from time to time.
What your background should look like

EXPERIENCE AND EDUCATIONAL REQUIREMENTS:
  • A Bachelor\xe2\x80\x99s degree or College Diploma, in the healthcare industry;
  • 5 years of clinical experience preferred;
  • Experience in case management, pharmaceutical or insurance would be an asset;
  • Bilingualism (French and English) is required;
MINIMUM SKILLS, KNOWLEDGE AND ABILITY REQUIREMENTS:
  • Candidate must have strong communication and interpersonal skills, including the ability to communicate with patients in a healthcare setting on extremely sensitive topics;
  • Professional, courteous, efficient telephone manner;
  • Strong ability to multi-task;
  • Must be detailed-oriented and organized;
  • Ability to work under pressure;
  • Proficient in Microsoft Office applications (Word/Excel);
