Role Overview

The Home Support Care Manager will work within a team to deliver Home Support services to older adults and disabled adults and their families in the Vancouver metro area. The goal of the Home Support program is to keep clients at home as long as possible with care management that is timely, tailored to individual needs, creative, and supervised by professional geriatric specialists.

In this role, you will play an integral role in maximizing the JFS mission and vision by supporting the social, physical and emotional wellness of all people. The Home Support Care Manager reports to the Manager of Home Support.

What You Will Do

Intake, home assessments, ongoing monitoring of services, and supervision of a team of home support workers

Territory will primarily be in the Better at Home Kerrisdale, Oakridge and Marpole (KOM) area and may cover other geographic areas based on program needs

Responding to client inquiries and needs with a solution that is client focused

Home visits of clients and workers for assessments, introductions and ongoing supervision

Ongoing communication with clients to modify service needs, to clarify roles and responsibilities and problem resolution

Liaising with other community agencies such as Vancouver Coastal Health to advocate, coordinate and problem solve

Collaborating with the KOM Better at Home Coordinator regarding mutual clients

Reviewing weekly scheduling of Home Support staff to ensure accuracy and efficiency

Providing back up weekend and/or holiday on-call support in a rotation and occasional first on call duty as needed

What You Will Bring to the Job

Undergraduate degree in nursing, social work, gerontology or related experience in social services setting/health care setting

Minimum two years' experience working with seniors

Excellent assessment, communication, computer and organizational skills

Team player with a positive attitude

Driver's license and access to vehicle an asset

Competitive salary in the non-profit marketplace. The annual salary for this role is $36,400 for a part-time position; 25 hours per week

Vaccination Policy

JFS requires that all employees and volunteers are fully vaccinated against COVID-19. Anyone hired after January 1, 2022 will be required to show evidence that they have received at least one dose of the vaccine prior to employment, and we will require that they be fully vaccinated within 30 days of their start date.
