Position Summary
Responsible to coordinate and conduct client assessments, problem identification, service coordination, counselling, education, advocacy, discharge planning and organize and coordinate support groups for clients and families
Responsibilities
1. Provides Case Management services :
Provides crisis management, case counseling and follow-up plan as required, including liaising with the client's family/caregiver and/or other care providers
Works with clients and caregivers on individual, couple and family issues; and liaises with community partners including clients' physician and/or other health professionals and or volunteers as needed
Co-ordinates and monitors self-directed client caseloads with the multi-disciplinary team
Maintains accurate records in the care plan, progress notes and statistical reports
2. :
Organizes and coordinates support groups and related services
Organizes educational workshops through partnership with other agencies and/or health care professionals
Plans, implements, coordinates and facilitates support groups to enhance the adjustment of clients/caregivers
Supervises volunteers assisting in groups or individual support
3. Ensures client safety and staff safety in all aspects of responsibilities:
Works safely at all times and ensures the safety of self and others by preventing and reporting hazards
4. :
Provides departmental and organizational support
Participates with the supervisor and the team in the development of standards, policy and procedures and quality improvement
Attends meetings and participates in St Paul's activities as required
Education:
Post-secondary education in health and/or social services or equivalent work experience
Registered health care professional (ex. social worker, nurse, occupational therapist, etc.), an asset
Valid First Aid and CPR certificate an asset
Experience & Skills:
Knowledge of assessment, counseling, case management and group work
Sensitive to the cultural needs of clients from various ethnic groups
Excellent interpersonal, communication, decision-making and assessment skills
Ability to work independently and co-operatively in a busy multi-disciplinary situation
