The Role: Case Manager (Full-time)
Home Fire is a harm reduction, scattered-site housing first program for Indigenous identifying youth ages 16-24 who are experiencing homelessness. The Case Manager provides ongoing support and resources in a flexible, compassionate, encouraging environment, including being responsible for all aspect of the case management of the clients. This includes mental and physical health supports, employment and education, social inclusion and recreational activities, and in-depth work connecting clients with their natural supports.
This permanent position has a 37.5-hour flex-week, that's reflective of the needs of the clients, removing barriers to connection and support (days, evenings, weekends as needed, minimum 2 evenings a week). We encourage individuals who are passionate about empowering Indigenous youth facing homelessness; to apply.
Key Responsibilities
+ Provide comprehensive case management and coaching to clients, helping them navigate their transition from homelessness to stability.
+ Support clients in reconnecting with family and community supports, advocating for their needs, and connecting them to education, employment, and other resources.
+ Assist clients with boundary-setting, conflict resolution, and communication to ensure long-term housing success.
+ Manage crisis situations by de-escalating conflicts, developing safety plans, and providing intervention and support.
+ Collaborate with external agencies, community partners, and professionals to create client-centered plans that align with their goals.
+ Help clients build life skills, including resume writing and accessing educational resources, while encouraging participation in arts, recreation, and cultural activities to boost self-esteem and community engagement.
+ Maintain accurate records of case management activities, including client goals, progress, safety plans, and outcomes.
+ Complete client intakes, document case meetings, and write reports on critical incidents and progress.
+ Attend required training and development sessions to enhance skills and knowledge related to case management and client support.
+ Perform additional duties as required.
Education and Experience Expectations
+ Degree or diploma in Social Services or a related field, with recent case management experience
+ Minimum 2 years experience working with vulnerable populations.
+ Demonstrated knowledge of Indigenous cultures, traditional practices, and the impacts of colonization and intergenerational trauma on Indigenous individuals, families, and communities.
+ In-depth understanding of the needs of vulnerable populations and experience with homelessness system navigation.
+ Knowledge of harm reduction and trauma-informed care practices is an asset.
+ Strong communication skills (both verbal and written), problem-solving abilities, and a collaborative mindset.
+ Proficient in Microsoft Office and case management databases.
+ Valid Standard First Aid and CPR Level C certification.
+ Valid Class 5 Driver's License, reliable vehicle, and a clean driver's abstract (transportation for clients will be required).
+ Ability to provide a Criminal Record Check with vulnerable sector search and a Child Intervention Record Check.
Reporting Relationship
This position reports to a Trellis Program Manager.
