Case Manager
for our
Bridge to Home
program, supporting individuals living in the community through a strength-based, housing-first approach.
What You'll Do:
Support clients with goal setting, budgeting, advocacy, and community integration.
Maintain a manageable caseload (11:1 ratio) to ensure personalized attention.
Collaborate with landlords, agencies, and other case managers to identify housing and support options.
Assist with applications for housing, treatment, and income supports (AISH, Alberta Works).
Maintain accurate documentation using the HMIS database and contribute to program evaluations.
What You Bring:
A diploma/degree in Human Services (or related field).
2+ years' experience in case management or supporting vulnerable populations.
Knowledge of the homeless-serving sector, mental health, and substance use.
Strong interpersonal, communication, and problem-solving skills.
A valid driver's license, personal vehicle, and satisfactory criminal record check.
