The Role: Case Manager (Full-time)
The Case Manager provides flexible, compassionate, and client-centered support to youth, focusing on their unique goals and strengths. Working from a relational and inclusive approach, Case managers build positive, trusting connections with youth and their Family and Natural Supports (FNS) to foster long-term stability, cultural belonging, and well-being.
Each Case Manager manages a caseload and supports youth in key areas like mental and physical health, education, employment, housing, life skills, and social inclusion. They reduce barriers to services, advocate for access to system supports, and create safe, meaningful opportunities for youth to thrive. This includes facilitating connection and reunification with FNS, supporting youth through challenging transitions, and navigating complex systems.
A core focus of the role is helping clients reconnect with and strengthen their relationships with Family and Natural Supports (FNS) to build long-term stability, cultural belonging, and a sustainable support network.
This position offers a 37.5-hour flex week, which includes a
minimum of two evenings per week
, and may include
occasional weekends
, to reflect the needs of youth and families and remove barriers to connection and support.
Key Tasks & Responsibilities:

Client Support & Case Management
Build strong, non-judgmental relationships with youth using empathy, inclusion, and a strength-based, client-centered approach.
Support clients with goal setting, life skills, and navigating systems related to housing, employment, education, mental health, and addictions.
Advocate for access to culturally safe, identity-affirming resources, including 2SLGBTQIA+ and Indigenous supports.
Maintain accurate and timely documentation, including intakes, case notes, and goal plans.
Respond to crises and facilitate difficult conversations when needed

Family & Natural Supports (FNS)
Actively identify, engage, and strengthen relationships with youth's Family and Natural Supports.
Promote inclusive, culturally meaningful connections that support long-term stability and belonging.
Facilitate visits, communication, and involvement in case planning and celebrations.
Mediate conflict and support healthy boundary-setting between youth and family members.
Advocate for families and connect them to appropriate community-based resources.

Housing & Life Skills
Help clients (and FNS when applicable) find and maintain safe, affordable housing.
Support lease signings, move-ins/outs, landlord relations, and understanding of housing rights.
Provide financial literacy and budgeting support to build independence..

Commit to Truth and Reconciliation and upholding principles of equity and belonging
Knowledge and Skill Expectations:
A Diploma or Degree in a Human Services Field (equivalencies also considered).
2-5 years of experience working with individuals and/or families with a strengths-based and trauma-informed lens (lived experience considered & an asset).
Knowledge of brief therapeutic strategies, housing first, recovery oriented, crisis management, and/or Indigenous social work practices an asset.
Valid driver's license and reliable vehicle with appropriate insurance.
Can work Monday to Friday with flexibility to accommodate clients on occasional evenings and weekends.
Current Standard First Aid CPR-C, ASIST, and TCI considered an asset.
Able to obtain clear Criminal Check with Vulnerable Sector Search and Child Welfare Intervention Check.
Reporting Relationship:
This position directly reports to a Trellis 24/7 Program Manager.
