Jean Tweed is a leading, community-based organization that provides a supportive environment for women with substance use, mental health, and gambling concerns. Recognizing the diversity of women's experience in today's society, our programs seek to address a range of issues in a woman's life that may contribute to substance use and gambling concerns.

The Jean Tweed Centre is building capacity to transform the level of support for women with substance use, mental health, and gambling concerns.

The Bathurst Housing for Women and Children offers affordable, permanent housing to 10 women-led families with up to 2 children in their care. This program is for women who are homeless or marginally housed, and who also identify substance use and/or mental health concerns. They will be supported by case managers who will provide intensive support on issues related to substance use and they connect women with other important supports such as primary health care. Women may also take part in a variety of groups offered by the case managers.

The successful candidate will provide encouragement, emotional support, help and advice on practical matter to clients, their families and communities. Coordinates support and outreach activities that may include: coordination of substance use support and related services, motivational counselling, brief assessment and referral, conducting education and information services, monitoring and supporting daily activities of clients.

Location: 4085 Bathurst Street, Toronto, ON M3H 3N9 (expected to work at multiple sites across the GTA)

Hours of Work: 22.5 hours per week, Saturday, Sunday and Monday 10:00am - 6:00pm

Responsibilities



As the Case Manager for Bathurst Housing, you will:

Lead a collaborative wrap-around process to ensure comprehensive service coordination that reflects individual needs. Provide case management support including counselling, outreach, on-going assessment, goal planning, referral, development of support systems, system navigation, etc. Provide referrals and support to access health and community-based services. Complete required case notes, evaluations, and data entry reports in an accurate and timely manner. Conduct job responsibilities with a commitment to harm reduction, trauma- informed, women-centred and anti-oppressive practices.

Service



Establish a comprehensive, wholistic, individualized service plan with each client on caseload. Lead a collaborative wrap-around process to ensure comprehensive service coordination that reflects individual needs. Provide case management support including counselling, outreach, on-going assessment, goal planning, referral, development of support systems, system navigation, etc. Provide referrals and support to access health and community-based services. Support activities geared towards establishing and maintaining healthy lifestyles with emphasis on primary health care, mental health, relapse prevention, community engagement, capacity-building, conflict resolution, crisis management and life skills. Support clients in self-advocacy and advocate on their behalf in the navigation for social services and access to health care systems. Develop and maintain relationships with other service providers and community resources. Participate in existing community and service networks. Where relevant work support clients in maintaining tenancy. Participate in the on-call rotation as required.

Qualifications



Bachelors Degree in Social Work, or a related discipline. A combination of 3 years of work experience in the field of Mental Health, Substance Use, and/or related field. Expertise in providing counselling and case management to individuals who are experiencing or have experienced a complex history of mental health concerns, substance use, and/or homelessness. Working knowledge of best practices in mental health, substance use, harm reduction, trauma and the therapeutic relationship. Working knowledge of trauma-informed practice, women centred principles and best practices for women and substance use. Working knowledge of housing, health care and community resources in the Toronto area and related provincial resources. Ability to work independently with minimal day-to-day supervision. Crisis intervention skills, with well-developed problem-solving abilities and interpersonal skills.
