Our current Case Manager will be going on maternity leave. This is a fixed term position for 12-months.
We are seeking a dedicated and professional Case Manager to manage cases for several of our foster homes in the Barrie area, with one home located in Mississauga. This role involves a blend of working from home, traveling to visit foster homes and children, and attending occasional in-person meetings.
Key Responsibilities:
Documentation:
Ensure foster parents complete and submit necessary documentation on time.
Compliance:
Verify that foster homes comply with Ministry standards.
Support:
Assist foster homes in navigating the child welfare sector and accessing community-based resources for children in care.
Communication:
Maintain weekly contact with foster homes via phone.
Visits:
Conduct monthly visits to foster homes, including private interviews with children every three months.
Assessments:
Perform annual assessments of foster homes.
Reporting:
Generate various reports, including Plans of Care, Safety Plans, Assessments, Special Rate Agreements, Incident Reports, Serious Occurrence Reports, and Case Notes.
Coordination:
Coordinate staffing with third-party providers, including scheduling and managing incoming documentation.
Home Studies:
Create Home Studies using the SAFE format (official training provided).
Training:
Train all caregivers in Therapeutic Crisis Intervention (official training provided).
Team Meetings:
Participate in weekly conference calls with the Care Services team.
Qualifications:
Education:
Bachelor of Social Work or Social Service Work Diploma.
Professional Membership:
Must be a member in good standing with The Ontario College of Social Workers and Social Service Workers.
Work Ethic:
Strong work ethic with robust work references.
Technical Skills:
Proficiency in Adobe Acrobat, Adobe Sign, and Microsoft Word.
Organization:
Highly organized and task-oriented with the ability to work under pressure and meet deadlines.
Communication:
Excellent clear communication skills with a friendly yet firm attitude.
Professionalism:
High standard of professionalism; absenteeism or use of rude/inappropriate language is grounds for immediate dismissal.
