Position Overview

The Case Coordinator will determine program eligibility and assess the need for care at home or in a personal care home for the elderly or infirm adult. He/she will coordinate the delivery of a broad range of professional and non-professional services based on the Case Coordinator Guidelines. Acts as a liaison and maintains communication with agencies and facilities involved with the client group.

Experience

• Two years directly related community experience preferred.
• Previous experience as a Case Coordinator in Community Health Services preferred.
• Experience with EFT implementation considered as asset.

Education (Degree/Diploma/Certificate)

• BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD, or related health/human service degree required.

• B. Serv. soc., ergoth\xc3\xa9rapie, physioth\xc3\xa9rapie, B. Sc., IPS, inf. aux. aut., B. Sc. inf., inf. aut., orthophonie, di\xc3\xa9t\xc3\xa9tique, ou dipl\xc3\xb4me connexe en sciences de la sant\xc3\xa9 ou en services sociaux, requis.

Certification/Licensure/Registration

• Valid driver\'s license and a vehicle.
• Licensure/registration as per professional designation required.
• If successful applicant is a Registered Dietitian, must be a graduate of a recognized dietetic internship program accredited by Dietitians of Canada.

• Poss\xc3\xa9der un permis de conduire valide et une voiture.
• Titulaire d\'un permis ou d\'une autorisation d\'exercice dans sa profession.
• Si la personne retenue est di\xc3\xa9t\xc3\xa9tiste, elle doit \xc3\xaatre dipl\xc3\xb4m\xc3\xa9e d\'un programme de stage en di\xc3\xa9t\xc3\xa9tique reconnu par Les di\xc3\xa9t\xc3\xa9tistes du Canada.

Qualifications and Skills

• Must be able to speak, read and write French at the predetermined linguistic level.
• Knowledge and experience with computers required.
• Effective oral and written communication skills.
