Job Summary


We are seeking a compassionate and dedicated Caregiver/Personal Support Worker (PSW) to provide exceptional care and support to individuals in various settings, including home, assisted living, group homes, and long-term care facilities. In this vital role, you will assist clients with daily activities, monitor their health status, and ensure their comfort and safety. Responsibilities



Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support Monitor patients' health by taking vital signs like blood pressure, pulse, temperature, and respiration; report any concerns promptly Support medication administration following care plans and HIPAA privacy regulations Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Use specialized equipment such as Hoyer lifts or resident lifts to safely transfer clients with mobility challenges Conduct patient observation to identify changes in condition or behavior, especially in dementia or memory care cases Maintain accurate documentation of care provided, patient progress, and incident reports in compliance with healthcare standards Perform light cleaning duties to ensure a safe and hygienic environment for clients Collaborate with healthcare teams including nurses, social workers, and therapists to develop or update personalized care plans Assist with transportation to appointments or community activities as needed Provide emotional support and companionship to promote mental well-being

Requirements



Valid certification in First Aid and CPR from recognized organizations Prior experience working in assisted living facilities, nursing homes, group homes, or home & community care environments is highly preferred Experience working with individuals with developmental disabilities, autism spectrum disorder, Alzheimer's disease, dementia, or other memory impairments Knowledge of HIPAA regulations ensuring client confidentiality at all times Ability to lift heavy objects safely using proper techniques; experience with Hoyer lifts is a plus Strong patient observation skills to monitor health status effectively Excellent communication skills to interact empathetically with clients and team members Ability to work independently as well as part of a multidisciplinary team Flexibility to work various shifts including evenings, weekends, or holidays as required A caring attitude combined with energy and motivation to improve clients' quality of life
