We are currently looking to hire a:

Caregiver Wellness Support - Social Worker



1 - Full Time, 6 Month Contract (35 hours/week)



Reporting To:

Community Health and Support Services Manager

Purpose:



The Caregiver Wellness Support Social Worker is responsible for supporting community members who provides unpaid physical, emotional, and/or practical support to a family or friend. The work of the Caregiver Wellness Support social worker is to coordinate caregiver wellness supports and provide client intervention and assistance to caregivers. The Social Worker is responsible for the intake, assessment and service coordination of clients coping with the responsibility of caring. They will be involved in case management and case consultations with other health professionals in Scarborough. The Social Worker matches caregivers to appropriate supports, services, and workshops/groups and is responsible for timely data collection, on-going client contact, and supporting other areas of SCHC.

Key areas of responsibility:



Receive and finalize intakes/referrals received. Utilize support of CSS Intake team as needed to better serve clients and their families Perform psychosocial assessments, therapeutic intervention/counseling, and referrals for caregivers both internally and externally with partners. Maintain accurate and concise confidential EMR documentation pertaining to client history and encounter data. Provide holistic and comprehensive case management services in collaboration with an inter professional team. Support and enrich services offered at ADC for client caregivers. Conduct home visits and provide comprehensive psychosocial assessments and counseling support. Compile statistical data from active caseload and from Scarborough Caregiver Consortium (CARG) accurately using CIMS database. Chair the Scarborough Caregiver Consortium, keeps records of the CARG sessions, review program plans and forwards yearly funding requests for approval. Facilitate caregiver support groups. Establish and maintain ongoing liaisons with other agencies and community resources that are relevant and necessary to the integration of SCHC client centered programs. Establish partnerships with other agencies and stakeholders to enrich programming and shared knowledge. Coordinate in conjunction with other CARG partners three yearly community educational sessions for caregivers in the Scarborough area. Provides timely intervention to clients in situations of abuse in consultation with Scarborough Elder Abuse Network and Community Relations Officer/Toronto Police. Participate in team meetings and provide input and ideas for improving services. Participate in QI initiatives and incorporate client feedback into practice and program development Participate in organizational committees. Conduct chart audits when required. Perform other duties as assigned that are reasonable within the scope of the job.

Scope and Accountability:



Work within aninterprofessional team setting. The Social Worker works closely with the, Caregiver Wellness team as well as other team members, to monitor and evaluate service delivery, as well as program policies to ensure program is meeting its objectives.

Educational and/or Professional Qualifications:



Bachelor's Degree in Social Work (BSW). Graduate degree in Social Work from a recognized University (i.e. MSW) is an asset. Registration and in good standing with the Ontario College of Social Workers and Social Service Workers (candidates who demonstrate ability to register with the OCSWSSW may also be considered). Experience in the field of Gerontology.

Level of Experience:



Three to five years of experience working with caregivers an asset. Experience in program development, implementation, monitoring and evaluation. Counseling experience in a community setting. Strong computer literacy skills.

Skills and Attributes:



Knowledge and understanding of the aging process, the specific needs of seniors, caregivers and adults with disabilities (this includes knowledge of/experience with cognitive impairments, and dementias). Demonstrated ability to provide Person and Family Centered Care. Demonstrated engagement in professional development and life-long learning. Proven proficiency in professional documentation practices according to the college standards. Strong organizational, communication, facilitation skills. Strong case management, assessment and counseling. Ability to work within an interprofessional team setting. Highly developed presentation and group facilitation skills. Skills in program development, implementation and evaluation. Current knowledge of all relevant resources (within and external to the community) for referral purposes. Capacity to strongly develop and maintain professional links and associate agencies, health care professionals and key stakeholders in Scarborough. Sensitivity to and awareness of cultural, racial, socio-economic diversity within the community. Oral and/or written fluency in other languages relevant to the community the program serves is a definite asset. Proficiency in the use of computers and various software applications. Current certification in CPR or willingness to become certified.

Significant Working Conditions:



Flexibility of hours- evenings and weekend work will be required. Ability to work from several organization locations due to program structure. Valid Drivers' License to drive in Ontario and access to a vehicle would be considered an asset. Subject to a Police Reference Check.

Remuneration:

$34.03 - $40.99

Band: D

Remuneration: $34.03 - $41.60*
