We are seeking a highly motivated, energetic, and experienced Career Practitioner / Case Manager / to join our team!
The Career Practitioner / Case Manager will work cohesively within a team of employment services professionals. The successful candidate will be responsible for the delivery of both in-house and outreach services to our clients; will be highly dynamic, organized and detail oriented, with outstanding people skills, highly adaptable.
The Career Practitioner / Case Manager is responsible for assessing an individual's employment/career needs and identify those who are suitable for case management. Case Managers work with these individuals in a collaborative, client driven process to develop a Return to Work Action Plan (RTWAP). The actions of a Case Manager must address the specific needs of the client as documented in their goals. The Case Manager must support accountability by organizing service delivery that is appropriate, coordinated, and timely to address client needs and desired outcomes.
Qualifications:
Preference will be given to individuals with a bachelor's degree/post-secondary education in Sociology, Psychology
,
Education, Human Services or Human Resources, or equivalent education and work experience
Several years of relevant experience in services related to career development working with diverse clients
Career Development Practitioner certification is considered an asset (CCDP)
Must have significant demonstrated experience in facilitating learning group activities, workshops
Knowledge of community organizations, service providers, and referral agencies
Experience working with persons from diverse background with varying barriers to employment.
Ability to access and effectively use information and communications technology
Responsive, proactive and strong problem-solving skills
Adherence to inclusionary practice, ethical decision making and respect for diversity
Must have a valid Driver's License or access to reliable transportation
Effective oral and written communication skills; effective communicator with the ability to support, motivate and build a trusting rapport with clients that support acceptance of individuals' values and employment goals.
Ability to maintain confidentiality during all aspects of employment
Ability to maintain positive relationships with clients and co-workers
Must have a recent, clear Vulnerable Sector Check or be able to provide one
Bilingual (French) considered an asset
Duties:
Under the direction of the Executive Director and Case Manager Team Lead, conduct client needs assessment and identify barriers to employment
Develop a plan to address barriers; monitor and evaluate the plan; adjust as needed
Provide group facilitation services; follow-up and support
Maintain relationships with employers, clients, service providers, community organizations, and other community members
Provide information about employment, training, and education options
