As a

Care Supervisor, PSS

within our CBI Home Health team, you will lead a group of direct Personal Support Services providers to ensure that clinical service delivery meets or exceeds standards set by CBI Home Health external stakeholders. Showing the way as a Regulated Health Professional (RPN or RN), you will develop and customize Personal Support Services plans to meet the patient's individual needs, as well as supervise direct care providers (field staff), unregulated staff (delegating tasks) or regulated staff (RPN) in the provision of services that align with their scope of practice, with a focus on ensuring service and clinical excellence.

As a Care Supervisor, PSS, you will:




Complete a comprehensive initial and interim care assessment within expected timelines as per the contractual agreement utilizing tools, forms, and processes as aligned with CBI Home Health policy and procedure, guidelines, and documenting all required information as well as any information pertinent to the client status and care planning. Ensure patient and care provider safety by using the risk assessment tools provided. Ensure service plans meet the needs of the patient and the care provider has the skills and experience to fulfill the plan of care. Reassess patients at interim visits to ensure the service plan continues to be appropriate Respond to client complaints, striving to resolve when able and escalating as required internally (to management) and externally (to funders) as appropriate. Identify opportunities to improve service and/or service delivery, engaging appropriate team members to drive effective and efficient change as required. Ensure patient experience is satisfactory and ensure patient is provided with all required information relevant to their care. Leading by example; promoting service delivery excellence through effective onboarding, mentoring/coaching, and managing performance of field staff (including performance reviews). Ensure staff receive mobility training. Liaise with administrative staff to ensure all care providers have completed CBI Health Annual Training Modules. Ensuring Performance Evaluations are up to date and uploaded in UKG for all staff. Ensure all care provider team members are fulfilling accountabilities as per their job description and are meeting expectations. Actively participate in maximising staff utilization, retention, and engagement expectations of field staff/care providers. Respond to identified staff training needs with action plans to resolve gaps, minimize risk, and ensure service delivery excellence. Collaborate in Workplace injury (WSIB, non-occ) processes (initial contact with injured staff, offering modified work where applicable, create incident report in Cority, advise administration staff to follow up on LOA's. Collaborate with Disability Management team in collecting required information (medical notes and/or FAF). Support and guide students when applicable. Follow-up on VSC renewal status for employees when escalation is required Ensure all reporting (internal and external) meets or exceeds all standards and expectations of CBI Health, stakeholders, and the associated regulatory body (if applicable). Ensure appropriate documentation is completed accurately and on time, including patient charting as per associated regulatory body and funder agreements. Effective communication (aligned with appropriate consent) with patients, families, and stakeholders (such as funders and other care providers) is consistent and documented. Actively identify risk and work with all stakeholders to mitigate and/or manage risk with particular attention to missed visits and clinical care. Ensure a timely and comprehensive response to Incident Reporting (IR) and investigation to address patient and Staff Safety Incidents and Patient Complaints. Ensure the execution of policies/procedures reflect CBI's best practices. Consistently meet KPIs related to role/field staff service delivery. Positive customer service experience: Client satisfaction scores meet expectations. Complaints/concerns of patients, families, and stakeholders are resolved or escalated appropriately. Successful staff retention strategies - turnover meets targeted rate. Employment engagement scores of Field Staff meet or exceed targets. Field Personnel: contribution to workforce planning leads to appropriate staffing levels. Participation in hiring (interview/selection), orienting, and onboarding contributes positively to retention and staff performance. Regular documentation and reporting audits to support that reporting meets or exceeds expectations. Reports, internal and external, meet timelines and standards consistently. Identify opportunities to improve service. Response to IR meets expected timelines and successfully minimizes or mitigates risk.
KPIs related to field staff service delivery are successfully and consistently met.

To take on the role of Care Supervisor, PSS, you must:




Possess a current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse or Registered Practical Nurse Have 2 years of supervisory experience and/or experience in the home health service delivery Be able to establish and build rapport with clients and Care Staff effectively Possess a valid Ontario driver's license with access to a reliable vehicle Computer skills - Proficiency with Microsoft Office Suite, ability to use and utilize scheduling systems Have the ability to provide the best possible solutions to problems that align with policies and uphold best practices Be self-motivated with an ability to work both independently and as an effective member of a team
