In this dual role you will be responsible for both coordinating exceptional in-home care and building strong community relationships. You will work closely with senior leadership to identify high-value opportunities, build strong partnerships, and drive meaningful growth across Right at Home Toronto. This position is ideal for someone who is energized by building relationships, shaping regional strategies, and making a measurable difference in the lives of Canadians and their families.
Key Areas of Accountability:
Strategic Growth & Community Engagement
Develop and implement short and long-term growth strategies to strengthen client relationships, expand referral networks, and meet organizational revenue goals.
Identify, pursue, and evaluate new business opportunities, emerging markets, and industry trends to drive sustainable growth.
Build, nurture, and assess strategic partnerships with community organizations, funders, and referral sources to support long-term collaboration.
Maintain accurate forecasting, reporting, and pipeline management in the CRM to support strategic decision-making.
Collaborate with internal stakeholders to provide insights that inform service offerings, capacity planning, and overall business development strategy.
Care Planning, Supervision & Support
Respond to inbound care inquiries, conduct client assessments/couch consults, and guide clients and families through care options.
Engage clients and families in the care-planning process and maintain individualized Care Plans that evolve with client needs.
Coordinate schedules and service delivery with operations teams while ensuring clear communication with the Care Team.
Monitor and supervise care delivery to ensure quality, safety, compliance, and timely resolution of urgent concerns.
Maintain accurate and up-to-date client and employee documentation within AlayaCare or other required platforms.
Recruits, supervises, and supports Care Team members.
The RIGHT Qualifications:
3-5 years of progressive experience in business development, healthcare management, or community engagement.
Background in nursing (RN/LPN or internationally trained nurses), social work, or gerontology considered a strong asset.
Experience in community healthcare, care coordination, and care planning, including assessing needs and developing individualized plans.
Familiarity with Ontario Health atHome and the role of care coordinators in patient assessment.
Strong networking, relationship-building, and partnership development skills.
Excellent communication, interpersonal, and negotiation abilities.
Strategic thinker with the ability to assess opportunities and shape growth initiatives.
Solid understanding of market trends, industry dynamics, and financial impacts of partnerships.
Proven project management skills with the ability to manage multiple priorities simultaneously.
Demonstrated leadership skills with the ability to guide cross-functional collaboration and influence stakeholders.
