A Day In the Life:



There may be no such thing as a "typical day" in the life of a Care Manager, but if there were, it might begin like this:

You're sitting at your kitchen table, coffee or tea in hand, the early light creeping in through the window. You take a moment to scan your emails, check in with the team, and review the day ahead. Your schedule is full, but your purpose is clear. You're here to create more moments that matter for families facing one of the most important decisions they'll ever make.

Your first visit of the day is with a family considering care for their mother, who's recently had a fall. They're worried and overwhelmed, but that's where you shine. You arrive not just with expertise, but with calm. You listen deeply, noticing what's said and unsaid. You ask thoughtful questions, read the room, and create space for emotion.

You offer solutions tailored to the client's unique needs, rooted in compassion, experience, and a deep understanding of what quality care truly looks like. You bring clarity where there's confusion, confidence where there's doubt, and peace of mind to a concerned daughter who hasn't slept well in weeks.

Mid-morning, you pull over safely and sign in to the company-wide daily huddle. It's a quick seven-minute touchpoint where the whole team connects to share good news, celebrate milestones, recognize our Heroes, and get aligned on the top priorities for the day. It's a small moment that keeps the entire organization feeling close, energized, and moving in the same direction.

Throughout the day, you might connect with our Care Hub about scheduling, follow up on a new client's care plan, or touch base with a Hero in the field to offer support and guidance. Duties & Responsibilities:



Completing Care Consults / nursing assessments to prospective customers and following up as necessary to promote our services and convert to new customers for the North Shore and Burnaby areas (note: geographic location may change as business needs change). Scheduling and attending initial meetings with seniors and family to educate them on our Hero Home Care solutions, converting families to our services. Managing the health and happiness of your customers by ensuring they have an accurate and up-to-date care plan that is being delivered by your team of Caregivers . Leading your team of Caregivers to perform to the best of their abilities, feel fulfilled in their work and deliver premium care to your customers. Liaising with your Care Support team to communicate any changes to care and ensuring that customer coverage is secured and assisting with scheduling as needed. Visiting independent living and assisted living facilities to impress upon them the benefits of Hero Home Care. Scope of responsibilities as per LPN or RN certification

Position Requirements:



2+ years experience as an Licensed Practical Nurse (LPN) or RN in the healthcare field in the Greater Vancouver Area 2+ years experience in a managerial role Valid BC Class 5 driver's license and current drivers abstract Access to a vehicle and comfortable driving throughout Greater Vancouver Are comfortable using various CRM programs and smart device
