Stay At Home Nursing Inc. is seeking a compassionate, detail-oriented

Care Designer

to lead the planning and coordination of personalized home care services for clients across our service region. As a key clinical team member, the Care Designer plays a critical role in conducting in-depth assessments, developing individualized care plans, and ensuring that care delivery aligns with client needs, preferences, and health goals. This role bridges clinical expertise with client advocacy to ensure quality outcomes and satisfaction.

Key Responsibilities:

1. Client Assessment & Care Planning



Conduct in-home or virtual assessments of clients' physical, emotional, cognitive, and social needs. Collaborate with families, caregivers, and healthcare professionals to design tailored care plans. Ensure care plans are culturally sensitive, goal-oriented, and aligned with best practices and regulatory standards.

2. Coordination & Implementation



Liaise with PSWs, RNs, RPNs, therapists, and other healthcare professionals to coordinate service delivery. Monitor service execution and adjust care plans as client conditions change. Ensure care plans are accurately documented in client health records and internal systems.

3. Quality Assurance & Clinical Oversight



Conduct regular follow-ups and reassessments to ensure care effectiveness. Review and evaluate clinical documentation, incident reports, and client feedback. Participate in internal audits and quality improvement initiatives.

4. Client & Family Engagement



Serve as the primary clinical contact for families seeking guidance or support. Educate clients and families on care strategies, safety, and chronic condition management. Advocate for client rights, preferences, and independence at all times.

5. Collaboration & Leadership



Work closely with scheduling and field staff to ensure coverage and consistency of care. Mentor field staff on care expectations, clinical standards, and person-centered approaches. Participate in interdisciplinary meetings and case conferences.

Qualifications:



Registered Nurse (RN) or Registered Practical Nurse (RPN) with current registration in good standing with the College of Nurses of Ontario (CNO). Minimum 3 years of clinical experience in home care, community health, or elder care. Demonstrated ability to develop and manage care plans. Strong communication, organizational, and interpersonal skills. Experience working with culturally diverse populations and complex care needs. Proficiency in electronic documentation systems and Microsoft Office Suite. Valid Ontario driver's license and access to a reliable vehicle.
