Oakview Home Care provides quality in-home care to individuals requiring assistance with daily tasks such as personal care, mobility, and feeding. All care is provided by trained Personal Support Workers or Nurses. Established in 2016, the company services clients throughout the GTA and Southern Ontario.

Summary

We are seeking a Care Coordinator to join our dynamic team at MyoHealth Rehab and Wellness Centre. In this role, you will play a vital part in managing patient care, ensuring seamless communication between patients and healthcare providers, and contributing to the overall success of our rehabilitation programs.

Responsibilities

Acts as main client contact and establishes care and scheduling requirements Follows up with clients and contractors to ensure service expectations are met Works with scheduling software to set up and monitor visits Matches clients with PSWs in their area Onboards new clients and PSWs Maintains client listing and PSW roster Verifies completed visits for payroll and billing purposes Acts as main contact for caregivers and clients Hiring and orientation of PSWs Accounts receivable maintenance and follow ups Liaises with Case Manager and other health care professionals

Requirements

Experience as a PSW Coordinator required Detail oriented Strong written and verbal communication skills Excellent interpersonal and problem-solving skills Strong administrative and organizational skills Proficiency in Word, Excel and Outlook Experience with AxisCare an asset Experience with MVA claims an asset Strong ability to follow up on issues or outstanding items
