We are recruiting a fulltime Care Coordinator - Office hours between 8:30am and 5pm weekdays.

Duties include:



Coordinate scheduling and management of Client and Employee files using our web-based software Engage with our frontline workers to match our clients' needs with their availability Gather necessary information to initiate client services/new intakes Provide on-going feedback to families regarding service delivery and continuous assessment of patient satisfaction using email or video conferencing Provide on-going support to staff regarding care issues that surface during service delivery Inform the Head of Operations of staffing issues (training, skills, performance, etc) that require follow-up Manage services between Equinoxe services and local community resources Update client and employee files daily (as required) Complete appropriate paperwork to comply with Equinoxe policy and procedures Compliance with Equinoxe LifeCare Solutions policies and procedures regarding confidentiality, dress code, ethical standards, and patient care
The ideal candidate for this role should possess a strong commitment to helping others and be able to work collaboratively with our team members. Candidate must have strong language skills (English and French).

Candidate must be "tech savvy" and be proficient in Microsoft Office. Educational level: DEC or higher Years of experience related to the job offer: 3-5 years relevant work experience

Description of qualifications:



- The successful candidate will be responsible, calm/patient, diplomatic and have a good, professional telephone manner and excellent customer service skills

- Attention to details and follow-up and ability to learn and use our care management software program are essential

- Prior experience in the healthcare sector is an asset

Nous recrutons un Coordonnateur(trice) de soins a temps plein - Horaire de bureau, du lundi au vendredi, de 8 h 30 a 17 h.

Responsabilites :



Coordonner les horaires et gerer les dossiers des clients et des employes a l'aide de notre logiciel en ligne Collaborer avec les travailleurs de premiere ligne pour faire correspondre les besoins des clients avec leurs disponibilites Recueillir les informations necessaires pour initier les services client/nouvelles admissions Assurer un suivi regulier aupres des familles concernant la prestation des services et evaluer de facon continue leur satisfaction, par courriel ou visioconference Offrir un soutien constant au personnel concernant les enjeux de soins rencontres pendant les services Informer la direction des operations des enjeux lies au personnel (formation, competences, performance, etc.) necessitant un suivi Assurer la coordination entre les services d'Equinoxe et les ressources communautaires locales Mettre a jour quotidiennement les dossiers des clients et des employes (selon les besoins) Completer la documentation requise conformement aux politiques et procedures d'Equinoxe Assurer le respect des politiques d'Equinoxe LifeCare Solutions en matiere de confidentialite, code vestimentaire, normes ethiques et qualite des soins

Profil recherche :



Le candidat ideal doit demontrer un fort engagement a aider les autres et etre capable de travailler en collaboration avec les membres de l'equipe. Une excellente maitrise de l'anglais et du francais est requise.

Le candidat doit egalement etre a l'aise avec les outils technologiques "tech-savy" et maitriser Microsoft Office. Formation : DEC ou plus Experience : 3 a 5 ans d'experience pertinente

Qualifications :



Sens des responsabilites, calme/patience, diplomatie, excellente communication telephonique et tres bon service a la clientele Grande attention aux details, capacite de suivi rigoureux et aptitude a apprendre et utiliser notre logiciel de gestion des soins Une experience prealable dans le secteur de la sante est un atout
