Position Summary

We are seeking a compassionate and highly organized Care Coordinator to support clients in navigating their care journeys. In this role, you'll manage internal and external coordination - handling referrals, follow-ups and care transitions - while also building trusted relationships with each client. This role is ideal for someone with a clinical or health navigation background who brings a detail-oriented mindset and a passion for advocacy and continuity of care.

Key Responsibilities



Act as a liaison between clients, specialists, hospitals, labs, and community providers Schedule, track, and follow up on specialist referrals, diagnostics, and procedures Support individualized care plans and participate in interdisciplinary case reviews Conduct follow-up calls and occasional external visits as needed Provide client education on diagnoses, medications, and care plans Ensure EMRs are updated with accurate appointment, referral, and outcome details Support insurance-related processes such as pre-authorizations and verification Serve as the main point of contact for care-related inquiries and coordination

Qualifications & Experience



Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario Bachelor of Science in Nursing (BSN), or equivalent health navigation background Other relevant backgrounds may include: patient navigator, medical social worker, health coach, or private care concierge Minimum 3 years of experience in care coordination, case management, community health or outpatient support Strong understanding of Ontario's healthcare system, pathways, and provider networks Exceptional communication, documentation, and organizational skills Proficiency in EMR systems and digital clinical workflows Additional training in chronic disease management is an asset
