Currently seeking a self-motivated healthcare professional with a passion for patient care to assume the contract role of Patient Support Program Coordinator. This contract position will require you to work from your own home or business office from which you will provide various independent services including being the primary point of contact when liaising with key stakeholders of the Patient Support Program, including: physicians, nurses, pharmacists, Program clinics, home service providers, private insurers, etc. As the Patient Support Program operates between 9:00 am and 5:00 pm , Monday to Friday; flexibility and availability of services during these times will be required. Services will include: \xc2\xb7 Facilitating enrollment of new patients into the Patient Support Program by obtaining referrals from prescribing physicians and collecting all pertinent information from the patient \xc2\xb7 Ensuring patient access (reimbursement) for the prescribed therapy as soon as possible by completing and submitting all necessary medical insurance forms \xc2\xb7 Providing progress updates and timely follow ups to physicians regarding all respective patients \xc2\xb7 Building trusting relationships with patients and their caregivers \xc2\xb7 Maintaining a professional level of verbal and written communication with all involved parties \xc2\xb7 Addressing and supporting issue resolution in a timely fashion \xc2\xb7 Maintaining detailed patient charts \xc2\xb7 Ensuring availability to participate in ongoing training, meetings with physicians, and education sessions as required The successful service provider will possess the following: \xc2\xb7 Registered health care professional with certification from a Canadian Regulated Health Professions College (e.g. Nurse, Pharmacist, etc.) \xc2\xb7 Experience working in Oncology \xc2\xb7 Technical knowledge of health care reimbursement from a patient and provider perspective \xc2\xb7 Proficiency with various technologies including but not limited to Microsoft Office, Outlook, and Web-based tools \xc2\xb7 Excellent verbal and written communication skills (French and English), and the proven ability to interact effectively with patients, health care professionals, and others within the care team \xc2\xb7 The ability to work independently and remotely with broader team members, seeking support as required \xc2\xb7 Exceptional relationship management and customer service skills (e.g. customer-focused, flexible, adaptable to change and the ability to work collaboratively with patients, physicians and nurses) \xc2\xb7 Problem solving skills with the ability to mediate and negotiate the resolution of conflicts \xc2\xb7 Strong attention to detail and organizational skills \xc2\xb7 Data analysis skills preferred Job Type: Fixed term contract Salary: $40.00-$60.00 per hour Schedule:
