We are currently looking to hire a: Behavioural Therapist - ABI Outreach Team1 Full-Time (35 hours/ week)Reporting to: Manager, Senior Support Services Purpose: We require an experienced Behaviour Therapist to work as a team member and case manager within the ABI Outreach Team. The ABI Outreach Team is an interdisciplinary team of professionals that supports people with an Acquired Brain Injury, as well as their families, in their homes and other community settings as needed. Key Areas of Responsibilities
Drive proactive continuity of care; collaboration/referrals with circle of care partners; assist seamless care transitions and provider interactions
Strengthen linkages in the delivery of a comprehensive individualized set of wrap around services
Decrease family/caregiver distress; advance care responsiveness; crisis navigation; reducing avoidable medical/ ER visits
Work with team members from a shared model of care to support clients living with an ABI in the community
Using the principles of Applied Behaviour Analysis, the Behaviour Therapist provides behavioural assessments, direct behavioural intervention/support as well as education to clients and their families
Provides direct interventions based on functional assessments and practical knowledge of ABA theories and its relation to individuals with an ABI
Provides support to clients/families using an adult education model and crisis prevention/intervention
Assess and evaluate client\'s home safety risks. Provide direct interventions to mitigate risks in independent living.
Educational and/or Professional Qualifications:
Bachelor\xe2\x80\x99s degree in Behavioural Science or Behaviour Analysis.
Level of Experience/Expertise:
Minimum 5 years of experience in community developing and/or delivering behaviour therapy services.
Previous experience supporting people with acquired brain injuries is an asset.
Thorough understanding of principles of Applied Behavioural Analysis
Skills and Attributes:
Experience in a community health setting and/or family practice setting preferred
Thorough familiarity with therapeutic methods and practices used with people with an ABI and/or cognitive disabilities.
Thorough understanding of health promotion and community development principles;
Experience in developing and evaluating client education materials;
Demonstrated commitment to principles of an anti-oppression and cultural competence framework;
Excellent interpersonal, communication (written and verbal), problem-solving, organizational and time management skills;
An interest and commitment to working in a low-income, multilingual, multiracial community;
Thorough knowledge of broad determinants of health and issues affecting low income, multi-lingual and radicalized and LGBTQ communities;
Computer literate (word processing, internet, data entry in Windows applications)
Demonstrated flexibility to work in a fast paced, ever-changing environment.
Ability to speak a second language of our priority populations is an asset
Community of Practice Partner Engagement:
Relationship building across inter-disciplinary teams and community partners-formalizing processes to strengthen efficient transitions across the care sector
Provide support to clients/families using an adult education model and crisis prevention/intervention
Provides consultation to internal staff members as well as community professionals from various organizations with the goal of enhancing skills and meeting client goals
Operations:
Utilize effective communications strategies; shared care plans; and EHR benefits
Meet performance indicators and participate in data collection
Significant Working Conditions: The successful candidate will be required to:
Conduct visits in the clients\xe2\x80\x99 homes, hostels, hospitals and other community settings working in sometimes difficult conditions, including hoarding and pest infestations
Work cohesively with staff members from across SCHC and partner agencies in accordance with the shared vision and protocols of the program
Travel throughout Scarborough for meetings at various sites and have access to a vehicle
Participate in both SCHC trainings and education, as required
Be available to work during the day; however, evening and weekend shifts may be required on occasion
