Position - Behaviour Support Coordinator- Registered Practical Nurse (Full Time)

Hellenic Home for the Aged Inc., a not-for-profit campus of care which provides multiple services at various locations within the Greater Toronto Area is currently seeking a passionate Behaviour Support Coordinator with excellent leadership, communication, and customer service skills, to work collaboratively with the Director of Care, Quality Improvement and Education Manager and Programs Manager, and the driven resident care team to provide an exceptional level of quality care to our seniors.
Reporting to the Director of Care, the Behaviour Support Coordinator coordinates the care and services for residents with responsive behaviours.
Responsibilities include (but are not limited to):
Utilizes effective communication and interpersonal skills when interacting with the resident, family members and internal and external care partners.
Delivers person- centred care which recognizes both the uniqueness of each resident and an awareness of ones' own contribution to the relationship, including personal attributes, values, and action.
Uses assessment tools based on best practices to conduct initial and ongoing assessments describing interventions with respect to the behaviours of the residents including:
? Documenting observational behaviours, recognizing that they have meaning, assessing and accounting for how the behaviours impact the care delivery process
? Assesses the meaning and identifies the behavioural risk using objective, systematic and holistic process that take into account the P.I.E.C.E.S.
? Identifies caregiver strategies that are resident-centred, age appropriate, maintains the resident's when responding to the behaviours and managing the associated risks.
Conducts behavioural rounds with the interdisciplinary team.
In order to join this dynamic and innovative team, you must have/be:
Must possess current Registration from the College of Nursing.
Must possess a Registered Practical Nurse Diploma.
Experience in Long-Term Care preferred.
Proficiency in interpersonal communication skills, both written and verbal.
Proficiency with WORD, email, internet use and computerized documentation.
Demonstrates knowledge in dementia care, delirium, mental health issues and the impact that is has on the resident's quality of life, family members and care partners.
Pay Rate:
$64,000 per year
Schedule:
10:00AM to 6:00PM and alternate weekends
