Position Summary
Behaviour Success Agent (BSA) will support the implementation of best practices across NSM hospitals. The BSA will improve care processes and standardize practice by providing clinical leadership and mentorship for staff in the assessment and management of older adults with cognitive impairment and responsive behaviour/personal expressions. Where capacity and time exists the clinical scope of the BSA will expand to include the focus to broader geriatric assessment.
The hourly wage will be determined in alignment with the pay scale of the applicable health care profession of the successful candidate.
PRIMARY RESPONSIBILITIES:

  • The BSA will support patients and health care providers at their hospital site in the assessment and management of older adults with cognitive impairment and responsive behaviour/personal expressions by providing direct patient consultation and knowledge translation activities to hospital staff. Where capacity and time exists, the focus will expand to a broader geriatric assessment.
  • Participate in a Community of Practice inclusive of representatives from the NSM SGS Program and other key stakeholders. Frequency and dates to be determined.
  • Update and implement the acute care NSM regional evidence-based best practice guideline for the care and management of older adults with cognitive impairment and responsive behaviour/personal expressions.
  • Based on the work of the BSA Community of Practice and in collaboration with the individual's hospital team, the BSA will:
  • Facilitate the advancement of a standardized approach to practice across NSM hospitals
  • Implement the regional guideline and applicable Behavioural Supports Ontario (BSO) resources.
  • Role model senior friendly care (delirium best practice, functional decline prevention, etc.)
  • Develop a site-specific sustainability plan.
Specific Responsibilities:
  • Advance the knowledge, skills and judgment of direct care providers through mentorship and knowledge translation activities (i.e. in-services, workshops, just-in-time).
  • Provide clinical leadership to hospital team(s).
  • Provide consultative case review support and, as appropriate, support/participate in the NSM SGS Complex Case Resolution process
  • Build strong collaborative relationships with key stakeholders and system partners.
  • Work collaboratively with the Community of Practice to adopt/implement and evaluate the regional guideline and BSO resources.
  • Work collaboratively with the Community of Practice and their hospital team to develop a site-specific sustainability plan that would define, at minimum, plans for ongoing leadership, staff development as well as plans for the sustainability of project initiatives.
Performs other duties as may be assigned by the direct manager.
Qualifications
Education:
  • Registered or non-registered health care professional with a diploma and/or degree required
  • GPA & PIECES training required
  • Completion of courses in geriatric care or mental health preferred
Experience:
  • Minimum 3 years recent experience in the provision of direct care and/or providing clinical leadership and support to direct care providers
  • Proven knowledge and appreciation in care of older adults with cognitive impairment and responsive behaviours/personal expressions
  • Demonstrated commitment to interdisciplinary care and evidence-informed practice
  • Ability to collaborate with others, mobilize a team and foster successful partnerships
  • Ability to strategically influence change,
  • Proven decision-making, critical thinking and conflict resolution skills,
  • Ability to work autonomously, and,
  • Proficient with computer programs like Microsoft Office Suite (i.e. Word, PowerPoint)
  • Demonstrates a participatory leadership style which includes approachability, visibility and accessibility.
