Responsibilities:




In this role as an Attending Physician, you will work collaboratively with the interdisciplinary team, including other APs, NP, RNs, RPNs, PSWs, to provide medical care for residents. In collaboration with the Medical Director, provides comprehensive primary care to residents. Collaborates and consults with the Director of Care, Nurse Practitioner, nursing staff, interdisciplinary team and external resources regarding residents' plan of care. Attends the home on a weekly basis to provide clinical care to assigned residents. Addresses resident acute and chronic care needs and provides appropriate and timely documentation. Identifies circumstances in which palliative and/or end-of-life care may benefit the resident and family members and work collaboratively with members of the interdisciplinary team. Participates in after-hour on-call schedule Conducts regular medication drug review and documents/updates residents' health records. Participates in outbreak management meetings and provides on-site medical oversight during outbreaks Commits to continuing education related to long term care, care of the elderly, palliative care and dementia care. Actively participates as a member of the Professional Advisory Committee. Participates in in-service education programs each year.




Qualifications:




Medical practitioner licence to practise medicine in the Province of Ontario, and always be a member in good standing of the College of Physicians and Surgeons of Ontario. Experience working in long-term care, preferred. Knowledge of regulations governing long-term care. Ability to work effectively as part of an interdisciplinary team Ability to build trust and interact in a professional manner with staff, residents, families, and other community partners.
