The Alzheimer Society of York Region ("AS York") helps improve the quality of life for people diagnosed with Alzheimer's disease and related diseases. We provide supportive counselling and education opportunities for individuals and families living with dementia. AS York also provides respite to caregivers and social activities for persons diagnosed with cognitive impairment through our D.A.Y. Centers A non-profit, community-based agency, AS York offers programs and works in partnership with several community organizations across the Region of York.
Reporting to the Senior Program Manager, this role is responsible for providing respite to caregivers while delivering At Home Adult Day Program (ADP) services to individual community clients in their homes. The work is carried out independently with limited supervision, requiring strong judgment, initiative, and the ability to adapt programming to meet client needs. Work is performed 3-4 days, minimum 21 hours per week up to maximum of 28 hours per week, Monday through Friday 8:00 a.m. to 6:00 p.m. Primary areas of service include, but not limited to, Eastern/Northern York Region (including Stouffville and Georgina), and mileage is reimbursed.
Key Responsibilities:
Meet with caregiver and client, to assess and develop initial care plan
Identify and advance opportunities for program that will improve service delivery and meet the specific psycho-social needs of the client
Implement a person-centred philosophy that provides a consistent and safe approach to care and service delivery
Implement and record client care plan in AlayaCare, and evaluate programs as required
Deliver one-on-one in-home programming to clients, by providing opportunities for meaningful engagement and motivation for the client; this could include activities such as music, exercise, creative endeavors, games, reminiscing, and crafts. Client-led engagement in household activities such as meal preparation, dishwashing, clean-up and laundry folding may be incorporated as part of care plan.
Assist with some activities of daily living i.e. assist with client toileting (no bathing), grooming (hair, makeup, buff/paint nails), medication reminders, support mobility needs, serve snacks/prepared meals for client (will provide supervision, will not feed client)
Monitor client behaviors and report any concerns to the Senior Program Manager and caregiver, Social Worker, as soon as possible
Report incidents/complaints as soon as possible to Senior Program Manager
Maintain client record and data collection in AlayaCare, including charting of client behaviors, incidents and issues
Calmly and professionally react to crisis situations
Maintain a safe and secure environment for clients
Maintain client confidentiality and high level of integrity
Comply with AS York Policies and Procedures
Follow organization's Occupational Health and Safety Program
Participate in training and education opportunities as required
Participate in AS YORK committees and workgroups as appropriate
Performs other duties as assigned
Qualifications:
At least one of the following: Personal Support Worker (PSW) certificate, Social Service Worker/Gerontology (SSW) diploma, Activation Coordinator/Gerontology diploma, Recreation Therapy/Gerontology diploma
Knowledge of dementia and current dementia care practice
2 years experience working with persons living with dementia
Well organized self-starter who can multi-task
Ability to prioritize and react calmly to crisis situations
Ability to maintain confidentiality
Strong communication skills, proficient in both written and spoken English
Second language is an asset
Computer literacy, proficiency in Microsoft Office (Word, Excel), AlayaCare,
U-First and GPA Certifications
CPR/First Aid Certification an asset
Food Handler's Certificate an asset
Clear Vulnerable Sector Screening Check
Valid G2/G Ontario driver's license and access to a vehicle are required
