Summary



As the Assisted Living Director at Haro Park Centre, you will play a pivotal role in enriching the lives of our residents by creating a caring and joyful environment. This position is essential in overseeing the delivery of high-quality assisted living services while ensuring compliance with health regulations and fostering a supportive community.

Role

Reporting to the Director of Care, the Associate Director of Care- AL Lead is primarily responsible for providing clinical support and guidance to the Long -term Care and Assisted Living staff to ensure each resident receives the appropriate ongoing care.

Responsibilities

Manages the clinical support provided to the

Assisted Living residents

by:

Providing day to day clinical support and guidance to the staff

Responding to emergency situations and reporting the outcomes to the Director of Care and other members of the Leadership team.

Communicating between departments to advise them of changes to residents that impact the services they provide.

Chairing monthly care reviews with staff and Vancouver Coastal Health.

Interviewing potential new residents to determine suitability for Haro Park.

Coordinating the move in/move out process to ensure the effective and efficient use of resources Completing appropriate paper work related to AL Occupancy Report such as; death reports, admissions/discharges and vacancies.

Maintaining ongoing communication with the Case Manager and VCH.

Reviewing and analyzing the annual Licensing audit report and developing an action plan to address areas of non-compliance.

Follow-up on resident serious incident reports.

Maintaining/overseeing the PSP, RCA daily routine(s) to suit Residents changing needs.

Orientation of new AL employees.

Facilitating student practicums

Ordering supplies (incontinence products, Melita products)

Ensures

Long-term Care residents

receive quality care by:

Attending staff huddles and passing on information as required.

Covering Resident Care Coordinator when away to chair care conferences, ordering medical supplies and overseeing the coordination of services such as dental, optometry, companion program, etc. as needed

Controlling the movement of beds to ensure prime occupancy levels are maintained by

Reviewing assessment packages for new potential residents being offered to Haro park Centre.

Conducting offsite reviews of individuals to collect collateral data on potential client's care needs.

Consulting and discussing new move-ins with ADOC/DOC.

Participating in teleconferences regarding residents who may pose as a challenge for Haro Park Centre.

Facilitating new move-ins including the confirmation of room turnover, facilitating internal transfers, and communicating to all team members on expected move-in date

Assisting Associate Director of Care - long term care (LTC) Lead with follow-up investigations of employee incidents and reporting outcomes to the Director of Care and Licensing.

Completes administrative support functions, including.

Engaging with the activities of Haro Park operations.

Participates as a member of the Leadership team.

Recruitment for the HCAP (Health Career Access Program)

Competencies

Knowledge of the Community Care and Assisted Living Act of BC.

Knowledge of VCH and licensing policies and procedures

Qualifications and Education Requirements

Registered LPN

Extensive experience working as an LPN in a seniors environment.

Computer literate/technically savvy

Preferred Skills Experience in a Leadership role Minimum of 2 years' LPN experience in LTC and or Assisted Living

Job Type: Full-time

Pay: $75,000.00-$80,000.00 per year
