Potlotek First Nation is seeking an Assisted Living Coordinator to oversee and deliver the Home Care Program for community members requiring personal support, safety monitoring, respite, light housekeeping, and meal assistance. This includes completing non-clinical home assessments to ensure the home environment is safe, appropriate support is being provided, and Home Support Workers (formerly Adult Care Workers) are completing their assigned task lists.
The Assisted Living Coordinator reports directly to the Health Director and works closely with Nursing staff to support appropriate client care. Any health-related concerns identified during non-clinical visits are communicated to Nursing so they can determine if further assessment or follow-up is required.
This position requires strong leadership, organization, communication, and the ability to work respectfully with families, community supports, and Health staff. The Coordinator plays a key role in maintaining high-quality, culturally respectful support services for elders, individuals with disabilities, and other members with identified needs.
RESPONSIBILITIES AND DUTIES
Oversee daily operations of the Home Care Program and ensure high-quality support services.
+ Supervise, schedule, support, and monitor Home Support Workers (formerly Adult Care Workers).
+ Ensure completion of mandatory staff training, including WHMIS/OHS, First Aid/CPR, Food Handler Certification, infection control, and other required training.
+ Conduct non-clinical home assessments, including safety checks, confirming task lists are being completed, and ensuring clients are receiving appropriate non-clinical supports.
+ Communicate any observed health concerns or changes to Nursing staff or the Health Director for follow-up.
+ Ensure Home Support Workers follow all policies, procedures, reporting requirements, and safety guidelines.
+ Support safe work environments and ensure safety of clients, staff, and community members.
+ Coordinate non-clinical personal support services such as light housekeeping, respite, meal support, and safety checks.
+ Clarify role boundaries and ensure Home Support Workers provide non-clinical support only.
+ Liaise with families, medical professionals, and external supports as appropriate.
+ Advocate for clients requiring additional services, equipment, or supports.
+ Verify staff timesheets and submit to Finance for payment processing.
+ Assist in recruitment, hiring, training, and orientation of Home Support Workers.
+ Maintain accurate documentation, including notes, incident reports, and non-clinical assessment records.
+ Prepare reports, updates, and recommendations for the Health Director.
+ Ensure compliance with PFN policies, privacy standards, safety protocols, and funding requirements.
+ Perform additional duties to support program development and the well-being of clients.
QUALIFICATIONS
Community College Diploma in Health, Community Services, Social Development, or a related field.
+ Two or more years of experience in home care, health care, or community support.
+ Experience supervising staff and coordinating program operations.
+ Ability to ensure staff meet mandatory training requirements.
+ Strong communication, documentation, and organizational skills.
+ Strong problem-solving and interpersonal abilities.
+ Proficiency in Microsoft Office Suite and email communication.
+ Current Criminal Record Check or Vulnerable Sector Check required.
+ Valid driver's license and reliable transportation for community visits.
+ Experience working with First Nations communities is considered an asset.
