The Fragility and Stability Program and Fracture Liaison Service (FLS) are supported by the Bone & Joint Health Strategic Clinical Network. The overall goal of the Fragility and Stability Program is to promote best practices for Albertans at risk of developing, or recovering from a fragility fracture, (usually indicative of osteoporosis). The Fragility and Stability Program focuses on the full continuum of care; prevention, hospital care for fractures, and care following a hip fracture; and includes evidence-based practices, protocols, and pathways for treating hip fracture, patient and family education, and new performance standards. The goal of the FLS is to improve osteoporosis care for patients who have suffered a hip fracture, highlight and address gaps within the care of this largely senior population, and ease their transition of care into the community. The Nurse Clinician is an important hub and champion for implementing and maintaining both the FLS at the Chinook Regional Hospital site and other Fragility and Stability Program work in the South Zone. Responsibilities for the FLS include identifying, investigating, and facilitating initiation of osteoporosis treatment (in conjunction with a Care of the Elderly Physician/Hospitalist) and ensuring follow-up of treatment recommendation for all patients presenting to a hospital with a hip fracture. The Nurse Clinician will link with community primary care providers to ensure a smooth transition of care from the FLS program back into the community. This is a leadership role with a high degree of autonomy to plan, implement, coordinate, and evaluate the initiatives. The position develops and manages a variety of activities to ensure successful execution of the initiatives while working collaboratively with the Fragility and Stability project team.

Your in-depth knowledge and skills, advanced judgment and clinical experience, will assist in providing solutions for complex health care issues at all levels with patients, interprofessional teams, administrators and policy makers. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
