Our Village of Erin Meadows, located in Mississauga, Ontario has an exciting opportunity for an ASSISTANT CLINICAL MANAGER - BSO to join their LONG-TERM CARE village team.

The Assistant Clinical Manager - BSO is responsible for providing essential health related advice and support to BSO care team of LTCHs across the region to serve residents through episodes of personal expressions / responsive behaviors that are unmanageable and deemed as a barrier to being able to receive care. This role will also support BSO designated residents\xe2\x80\x99 transitions across different care settings and link the receiving team with resources and stakeholders to ensure successful transitions. The position is based out of The Village of Erin Meadows with supervisory/managerial responsibilities for the Mobile Team and of dual accountability under direct management from ASP BSO leadership.

Key Accountabilities:

  • Understand and commit to the goals, framework, and principles of Behavioral Supports Ontario (BSO)
  • Work to full RN/RPN scope of practice with demonstrated extensive knowledge with various types of dementia, delirium, mental health, other neurological conditions, and effective skills in planning, implementing and evaluating behavioral support in accordance with ASP and LTCHs\xe2\x80\x99 established policies
  • Work collaboratively with the ASP BSO Clinical Manager for the recruitment and performance review of the mobile team and fulfil other supervisory/managerial responsibilities as determined
  • Act as the regional resource for BSO team of regional LTCHs, prioritize and handle referrals across the region and educate clinical team members in best practices for personal expressions / responsive behaviors to strengthen person-centered behavioral support
  • Assist/support clinical team members and BSO team members in a holistic approach to develop and implement behavioral care plan/strategies for referred residents
  • Review relevant documentation and if needed, observe the referred residents and their interactions in the various environments and at various times to support with the completion of the behavioral assessments and ensure that the supportive strategies are properly placed in a timely manner
  • Monitor and evaluate residents\xe2\x80\x99 progress against the behavioral care plan, suggest changes based on fluctuating needs and priorities, and provide written consultation/progress notes as appropriate
  • Participate in joint practice partnership with Mental Health and Geriatrics in the assessment of referred older adult clients who have complex medical, mental health and behavioral issues.
  • Identify and communicate gaps/risks, develop and implement initiatives with the team and leadership to optimize behavioral support
  • Promote and implement clinical practice standards and evidence-based best practice to improve care outcomes
  • Work collaboratively with external partners such as HCCSS, LTCHs, Senior mental health outreach team, specialists, pain counselor, community referral programs etc., and internal BSO team to enhance the quality of life of referred residents
  • Provide support to ensure the successful transition of referred residents of LTCHs across the various transition points, i.e., Acute Care, other facility-based care etc.
  • Facilitate transfer of knowledge into practice and enhance team members\xe2\x80\x99 ability to manage personal expressions / responsive behaviors and implement care plan interventions through role modeling, coaching, mentoring, non-pharmacological and pharmacological approaches
  • Participate in case conferences/meetings with clients, family, service providers, and members of the BSO interdisciplinary team for identification and management of the clients\xe2\x80\x99 personal expressions / behaviors as required
  • Participate in clinical team meetings, special projects, quality improvement activities and program development, as needed
  • Maintain accurate and timely monthly statistics through the identified Statistical Reporting System

To be successful as our new Assistant Clinical Manager - BSO, you will:
  • Hold a current registration as a Registered Practical Nurses (RPN) or Registered Nurse (RN) with the College of Nurses of Ontario
  • Have a minimum 3 years of experience working with older adults in long term care and the community
  • Minimum 3-5 years of experience working with the elderly with Alzheimer\'s Disease and other related dementias
  • Ability to plan care using Resident Assessment Instrument\xe2\x80\x93Minimum Data Set 2.0(RAI-MDS 2.0)
  • U-First, P.I.E.C.E.S, GPA Certification
  • CPR & First Aid Certificates
  • Intermediate to Advanced \xe2\x80\x93 Word, Excel, PowerPoint & Outlook
  • Strong written and oral communication skills
  • Self-directed, team player
  • Valid driver\'s license and access to a reliable car
