The Patient Care Coordinator, under the direction of the Manager, Adult Mental Health, leads the interprofessional
team to promote excellence in clinical care, staff development, patient and family?centred service, and a safe
therapeutic environment.
EMPLOYMENT REQUIREMENTS:
Education/Experience: Unencumbered registration with the College of Nurses of Ontario
Baccalaureate degree in Nursing
Minimum 5 years clinical practice with demonstrated progressive clinical leadership, clinical experience in Mental
Health is required. Certification in Psychiatric and Mental Health Nursing (C)anada (CPMHN[C]).
Current NVCI certification
Knowledge of practice standards and guiding principles to foster professional practice and accountability.
Experience in the development of clinical guidelines and protocols, promoting the use of research and
evidence?based practice to guide patient and family centered care is required.
Skills/Abilities: Demonstrate leadership abilities with high levels of critical and logical thinking, analysis and/or reasoning to identify
underlying principles, reasons or facts. Demonstrate high levels of initiative, resourcefulness, flexibility and adaptability.
Demonstrate commitment to patient and family?centred care.
Demonstrate ability to work effectively both independently and in a collaborative practice model as a member of an
inter?professional team. Demonstrate knowledge and understanding of the relevant acts, regulations, legislation and standards related to
Occupational Health and Safety in the hospital setting. Demonstrate excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point
and Outlook) and patient information systems. Demonstrate superior interpersonal and communication skills, both written and verbal.
Demonstrate time management skills with the ability to manage workload and adapt quickly to a fast?paced,
dynamic work environment. Demonstrate ability to assist clients with concern and empathy, respect their confidentiality and privacy,
communicate in a courteous and respectful manner, and consider intended audience. Demonstrate commitment to the safety of co?workers and patients.
Salary Range: $56.884 to 66.571
