The Emergency Treatment Fund (ETF)
is a funding initiative offered by Health Canada to municipalities and Indigenous communities to address urgent substance use needs.
The HOME (Health Outreach Mobile Engagement)
Team
is the Guelph CHC's contribution to ETF along with the Sanguen Health Centre, and the Mount Forest Family Health Team, which will provide mobile wrap-around support services to individuals struggling with substance use who face barriers to accessing traditional services. The HOME team will expand on existing programs to operate a mobile outreach vehicle in the City of Guelph, in tandem with Sanguen's Mobile Health Van in Wellington County, and offer additional place-based supports.
Reporting to the Guelph CHC Allied Health Supervisor, the HOME Team Administrative Assistant will play a pivotal role in keeping program operations smooth and organized. They will work with Integrated Program staff members to support the creation of the HOME program.
We are looking for an Administrative Assistant who:

Works from a harm-reduction philosophy and values team-based care
Thrives in dynamic environments and enjoys contributing to new program development
PRINCIPAL JOB DUTIES
Assist with scheduling meetings, tracking timelines, and maintaining project plans
Support communication between clinical teams, funders, and external partners
Maintain accurate records of program activities, decisions, and milestones
Serve as a point of contact for inquiries from staff, partners, and community members
Coordinate onboarding for new staff (e.g., credentials, access to systems)
Manage supplies, equipment orders, and facility setup as needed
Assist with data entry, tracking metrics, and generating reports
Support EMR setup, user access, and liaise with IT for troubleshooting
Support audits, evaluations, and continuous improvement effort
QUALIFICATIONS
Minimum Grade 12 diploma; post-secondary education in office, medical, or business administration is an asset
Proficiency with Microsoft Office software (outlook, word, excel)
Ability to collaborate across clinical, allied health, administrative, and external teams
Experience with Electronic Health Records is an asset
Strong written and verbal communication skills
Ability to set goals and get results within a set timeline
Ability to manage multiple tasks and work independently
Effective time management, decision making and organizational skills
High level of confidentiality
Flexible towards changing priorities, timelines, and program needs
