Addictions Case Worker

Job Description

This is a full-time on-site position.

Reporting to the Manager, DARE Program, the Addictions Case Worker enhances the dignity and quality of life of our clients through his/her assumed responsibility to assess, admit and provide a continuum of support services to clients in their transition to treatment as well as from treatment to independent living.

Roles and responsibilities include but are not limited to:



Responsible for the assessment, admission and orientation of clients applying for admission to the DARE Pre and Post Treatment Program including the explanation of program guidelines and expectations; conduct assessments and develop individualized service plans for pre-treatment clients, and develop housing, aftercare and relapse prevention focused plans for post treatment clients; facilitate group sessions and to empower clients to cooperate, work in groups and support each other with their individual goals; monitor clients' progress through regular follow-up, updates, crisis intervention and development of personalized relapse prevention plans; support clients in finding and maintaining housing by contributing to the enhancement of skill building activities for daily living; assist clients to consider options for leisure and social time, empowering them in building supportive networks to enhance independent living; provide supportive counselling and skills development in order to assist clients in preparing for interviews with landlords and employers; liaise or advocate with internal and external support systems to provide complementary services to clients; perform any other job related tasks that may from time to time be assigned by the Manager, DARE Program, or the Assistant Executive Director, or designate.

Skill, Nature and Scope:



Demonstrated commitment to the Mission, Vision and Values of GSM is a requirement. Bachelor of Social Work or a degree in a related field such as Community Work, Psychology, Social Services etc. and/or Certified Alcohol and Drug Counsellor through the Addiction Intervention Association. At least three (3) years' minimum experience in counselling and Aftercare work preferably in the field of addiction. A minimum of three years employment experience in individual and group therapy for individual in Aftercare. Extensive knowledge of employment and housing resources in the community. An awareness of the addiction-related community, public health and educational resources. Counselling experience and or knowledge of the following treatment modalities: Cognitive therapy, psychotherapy, harm reduction and the 12 steps. A well-developed knowledge of addiction, concurrent disorders and supporting recovery. A thorough understanding of homelessness, poverty, health, mental health and related issues is required. Solid experiences working with homeless individuals in conducting assessments, case management, crisis intervention, case coordination. Excellent group facilitation skills, preferably experience applying cognitive behavioural approach Strong interpersonal, problem-solving skills and ability to manage multiple tasks and prioritize work overload is essential. Excellent written communication skills to produce memoranda, correspondence and progress reports. Excellent interpersonal skills to maintain teamwork with the ability to communicate effectively with individuals from diverse backgrounds. Strong computer skills. Strong organizational and administrative skills to a high standard of professionalism
