The Corporation of the County of Huron's Homes for the Aged department is seeking a temporary full-time Activation Staff member for Huronview (Clinton)!Position Summary
The term of the contract is approximately 18 months.
Huronview and Huronlea are sister Homes in the County of Huron. These municipal homes provide health services for those who can no longer live independently in their own homes and require nursing and personal care 24 hours a day. Role and Responsibilities
Direct services to residents
Within the framework of supporting resident decision-making, choice, independence and wellness, the Activation Staff assists all residents in achieving or maintaining their needs, physically, emotionally, socially and spiritually.
Assesses the residents' needs through an activation assessment tool or admission, by interviewing resident/family or significant other.
Provides an orientation to the home and other residents.
Writes written goals for each resident, which is evaluated on a quarterly basis.
Attend care conference meetings as required. Summary notes are to be added to the Interdisciplinary notes prior to all Care Conferences.
Provides therapies and restorative care programs under the direction of the Homes' Doctor, Therapist (Occupational Therapist, Physiotherapist, Speech Therapist) and Charge Nurse.
Implements 1:1 and small group programs both in residents' rooms and in existing small lounges on either units in the Homes. This will include the use of Montessori and Sensory equipment on a regular basis
Implements and engage residents in stimulating large group programs in a large space adequate to accommodate the large groups comfortably.
Coordinates volunteers as required.
Observes, reports and documents in progress notes resident concerns and behaviors.
Drives and/or accompanies residents on various planned group outings.
Evaluates activities from feedback questionnaires, Residents' Council, resident attendance and interviews with residents, their families and representatives.
Communicates verbally and in writing with employees of the Homes, which demonstrates respect, privacy and confidentiality for each resident.
Performs other duties, as directed by the Programs and Social Services Manager.
Environment:
Helps to keep the Home program activity areas clean and tidy, which will promote safety in the workplace.
Safely uses and maintains equipment and supplies in the Homes.
Provides input into the budgeting process and in the purchase of necessary supplies as approved by the Manager of Programs and Social Services.
Creates posts and removes visual aids, posters, fliers, calendars within the Homes and on the Huron County website, decorations, etc., as per programs or season as required.
Organizational Responsibilities and Accountabilities:
The Activation Staff follows the Standards for Programs and Social Services as outlined in the LTC Act 2007 and Regulations l and the Policies and Procedures of the Homes.
Takes a turn at completing the administration portion of the activity calendar, while having regular input as a team member into the planning of the calendar.
Will have adequate knowledge of all activities planned on the activity calendar. In turn will be able to competently carry out all activities planned on the monthly calendar.
Attends and participates in department meetings offering input into developing, evaluating and revising department policies and procedures.
Collaborates with others in planning care, recreational and leisure programs according to the Residents Bill of Rights. Supports volunteers who assist with programs.
Personally follows the Homes' Policies.
Supports staff committees and is personally responsible for maintaining their own professional development in their career.
Demonstrates through his/her performance, a strong commitment to the philosophy of Long-Term care and the Mission/Vision of the Homes.
Participates in flexible hours, such as evenings, days and weekends.
