Status: TEMPORARY Full Time



Department: Ontario Structured Psychotherapy Program



Site: St. Joseph's Health Centre



Shifts: Days



Rate of Pay: $39.06 - $53.69

per hour





Reporting to the Manager - Ontario Structured Psychotherapy Program, the Access Point Facilitator will be responsible for processing referrals through Access Point Northwest, conducting intake eligibility screening, incorporating measurement based care and assigning clients to Service Delivery Sites, in the regional network, based on service needs.





Qualifications

:





Minimum qualification includes undergraduate degree and registered member in good standing with applicable professional college (e.g. College of Nurses of Ontario, Ontario College of Social Workers and Social Service Workers, Ontario College of Psychologists, College of Registered Psychotherapists of Ontario, College of Occupational Therapists of Ontario)

Knowledge and experience with referral processes, intake, assessment and triage

Experience assessing clients with mild to moderate mental health issues

Experience and understanding of the etiology and treatment of depression and anxiety disorders, as well as OCD and PTSD






Skills/Abilities:





Maintains an awareness of ethical guidelines and practices for the provision of effective client centred care Awareness and understanding of the diagnostic categories and criteria as listed in the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) Knowledge of relevant legislation, e.g., Mental Health Act, Personal Health Information Protection Act, Regulated Health Professions Act, Health Care Consent Act, and professional standards to ensure compliance and consistency in provision of client care System navigation and knowledge of community mental health resources Passionate about contributing to positive social change and commitment to anti-oppression and anti-racism Knowledge of a range of theoretical perspectives, psychosocial rehabilitation principles, chronic disease self-management, holistic wellness approaches, and an ability to work in an inter-professional team environment Demonstrated excellent interpersonal communication skills (oral and written) to effectively communicate with clients, referents, consultants, partners, program managers, and directors Demonstrated experience with technology, documentation in the Electronic Medical Record and working with electronic databases Time management and organizational skills in order to effectively address and prioritize multiple tasks and timelines in a fast-paced environment Excellent oral and written communication and interpersonal skills, judgment, critical thinking skills, problem-solving and time management skills Ability to work closely with community mental health service partners and Access Point Facilitators

Experience working with Indigenous people and other diverse ethno racial and cultural backgrounds is considered an asset Experience working with 2SLGBTQIA+ individuals is considered an asset



CONDITIONS OF EMPLOYMENT



This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
