Access and Flow Care Leader

to join our

Transitional Care

team based in

West

Bedford, Nova Scotia.




The transitional care model supports Nova Scotia Health patients/families who require additional time, services, and support to transition back to home/community, by providing an alternate space with intentional programming, building design, and resource allocation to increase the probability of returning to their previous living situation.


The Access & Flow Care Leader will collaborate with our partners at Nova Scotia Health to determine bed availability and assist with the coordination of admissions and discharges. They must ensure an interdisciplinary approach to the discharge planning process and ensure continuity of care for the patient during admission, through their stay and with their discharge. They must provide quality patient care consistent with applicable legislation, nursing regulatory standards, Accreditation standards, evidence-based practices, and Shannex's Vision, Core Values, Guiding Principles, Strategic Directions, and policies and procedures.

About the Opportunity




Promotes a Patient-directed philosophy while focusing on Home First principal Supports initial identification and assessments of potential admissions in consultation with NS Health Partners and Internal MRHCP Collaborates daily with NS Health Partners to ensure admissions and discharges are coordinated and vacancies are filled immediately, working closely with Access and Flow and Bed Management within NS Health Aids with complex discharge needs; collaborating with the interdisciplinary teams to coordinate discharge plans, transportation, care plans, medication management, etc. Assisting clinical teams with education for patients and families in preparation for discharges, liaising with community partners and the NSH Outreach team Supports clinical leadership, chairs daily rounds/weekly rounds and provides oversight of the patient documentation system and C3 tile system to ensure documentation is up to date and relevant Assures compliance with regulatory agencies and the accreditation process Establishes and maintains an organizational structure and professional model of practice that maintains effective communication, and clearly defines responsibility and accountability Assumes the duties of registered staff as required.

About You




Bachelor of Nursing, Bachelor of Science in Nursing preferred Current registration with the Nova Scotia College of Nurses Minimum 5 years nursing experience which includes two years clinical nursing with acute care nursing experience and 2 years' administrative experience or equivalent being an asset Desire to work in a progressive environment that embraces change A positive approach to new initiatives and opportunities for improvement Knowledge of the principles of coordination, organization, and administration Knowledge of current literature and applied research for the delivery of health care Knowledge of operational and personnel rules and regulations and the collective bargaining agreement Knowledge of departmental and provincial policies and practices affecting work Knowledge of West Bedford Transitional Care an asset Strong interpersonal skills and relationships with community partners and leaders
