What We Do
There has never been a more exciting time to be at WAHA. We are an integrated health system providing the full continuum of care to the six communities of the western James Bay coast with over 500 employees, physicians, and partners in care. As a health authority, you will work in a unique setting where you have the opportunity to care for patients in all phases of their life journey, in all areas of their health experience. What You'll Do
Lead the coordination and delivery of palliative and end-of-life care within the community setting
Provide clinical consultation to interprofessional teams for complex symptom management, pain control, and end-of-life support
Facilitate collaborative care planning and discharge coordination with community-based palliative programs and home care services
Serve as the primary liaison between WAHA and community-based palliative care teams
Develop and maintain referral and communication pathways to ensure seamless patient transitions
Lead palliative education initiatives for hospital and community health professionals
Deliver in-service training on symptom management, communication, and end-of-life care.
Lead or support palliative quality improvement initiatives and audits
Develop and update policies, pathways, and clinical tools consistent with provincial and accreditation standards
Promote family-centred, culturally grounded care that respects patient preferences and traditional values
Collaborate with Elders, Healers, and community partners to incorporate traditional approaches to care and healing
What Your Bring
Bachelor of Science in Nursing required
Registered with College of Nurses of Ontario and in good standing
Master's in Nursing, Leadership, or Palliative Care preferred.
Certification or advanced training in Palliative Care Nursing (e.g., CHPCA, LEAP, or equivalent) required or in progress
Minimum of five (5) years of recent clinical experience in palliative, oncology, or acute care.
Cultural knowledge and experience with integration of Indigenous traditions, healing practices, and medicines
Ability to communicate in the local native Cree language is an asset

$89,797.50 - $128,700.00
