Qualifications

Required:

Regulated Health Professional designation in good standing with the appropriate College. Minimum 2 years of experience supporting patients with chronic or complex medical needs. Strong communication, advocacy, and problem-solving skills. Demonstrated knowledge of community health and system navigation.

Preferred:

Training in palliative or chronic disease care models. Experience in patient education, care coordination, or health coaching.


Skills:

Excellent communication skills. Documented commitment to continuing education Demonstrated leadership, problem-solving, decision making, organizational, communication and interpersonal skills. Able to articulate adult learning principles and apply these in daily work to support patients, families and staff. Commitment to Patient Care & Safety and Service Excellence. Satisfactory vulnerable person's sector criminal background check.
